Project overview.

A coordinator’s workspace for organising the practical work around a patient’s transition from hospital to home. Patient context, task ownership, requests and scheduled reviews are connected in one desktop experience.

THE PEOPLE

Care coordinators working alongside ward teams, pharmacists, social workers and caregivers.

THE SCOPE

Overview, patient and team context, task board, requests, schedule, service insights and workspace settings.

THE STATUS

Design concept.
UX/UI and interaction design.

The problem.

Discharge coordination involves practical arrangements, task owners, requests and scheduled reviews. When those details are separated, coordinators have to reconstruct what is pending and who should act. Clinical clearance and practical readiness also need to remain distinct, so one is not mistaken for the other.

The solution.

Connect the day’s workload to patient-level context in a shared desktop workspace. Keep tasks close to the patient, responsible person, due time and status. Give supporting requests a visible queue, place reviews in a weekly schedule and distinguish clinical clearance from the practical arrangements needed for the transition home.

A few priorities.
A clearer direction.

Three priorities that guide the experience.

01

What needs attention today?

Bring reviews, pending work and open requests into the starting view.

02

Who owns the next step?

Keep the patient, responsible person and due time close to the task.

03

What is still being arranged?

Show practical readiness alongside the wider patient episode.

A place for everything.

Designed with intent.
Open to learning.

This is a coordinator-platform design study. Clinical evaluation has not been conducted. A useful next check would examine task ownership, pending arrangements and the distinction between clinical clearance and practical readiness.

  • Identify work needing attention today.
  • Find the owner and due time for an arrangement.
  • Read pending requests within patient context.
  • Distinguish clinical clearance from practical readiness.

Screen values are prototype data. Coordination metrics are not clinical outcomes or measured results from this project.
